Immigration physicals at Shiraz Medical Group are required by the United States Citizenship and Immigration Services (USCIS) to obtain a United States green card. Physicians designated as civil surgeons by USCIS can complete immigration/green card exams.

What should I bring to an immigration physical?

Bring the following with you to an immigration physical at Shiraz Medical Group:

• Form I-693 (don’t sign part 2)
• Previous immunization records
• List of medications
• Government-issued photo ID, such as a driver’s license, passport, or ID card
• Method of payment

What happens during an immigration/green card physical?

You’ll complete a medical history form when you arrive at Shiraz Medical Group for an immigration/green card physical. 

Your provider weighs you and checks your blood pressure, pulse, and additional vital signs. They also discuss your medical history, family medical history, medications, lifestyle, symptoms, and more.

During a physical examination, your specialist evaluates your overall health. They may assess your eyes, vision, hearing, throat, breathing, heartbeat, reflexes, abdomen, limbs, or other areas to screen for signs of a medical concern. 

You might need vaccinations and screening for tuberculosis, sexually transmitted diseases (STDs), or other diseases, as well as blood work, urine testing, X-rays, or other imaging procedures.

What happens after an immigration physical?
